We are seeking to recruit a Discharge Coordinator to join our team at Denmark Hill covering the medical and surgical wards.  The post will give the successful candidate an opportunity to develop a thorough understanding and knowledge base of the many discharge pathways and processes available to patients who required integrated (supported) discharge from an acute hospital bed. You will be working with, and learning from, the team of senior discharge coordinators as well as a wide range of clinical and non-clinical staff, including the multi-disciplinary team (MDT), and will become a vital contact point for ensuring the details of a patient's discharge is communicated effectively to all involved in the process. You will be a registered nurse, with the relevant post-registration qualification, and have some experience of the processes and pathways associated with complex patient discharges from acute care.

You will be able to build effective relationships with patients and families, all members of the MDT team and colleagues in social care, CCGs and community services. You will have the opportunity to experience many of the clinical specialties providing care at the Trust through your allocated caseload of up to 10 patients at any time. You will be expected to work independently,  creatively as part of the team and have the expertise to prioritise a busy workload.
