We are seeking to recruit a Discharge Coordinator to join our team at Denmark Hill.  The post will give the successful candidate an opportunity to develop a thorough understanding and knowledge base of the many discharge pathways and processes available to patients who required integrated (supported) discharge from an acute hospital bed. You will be working with and learning from the team of senior discharge coordinators as well as a wide range of clinical and non-clinical staff, including the multi-disciplinary team (MDT), and will become a vital contact point for ensuring the discharge of patients is communicated effectively to all involved in the process. You will be a registered nurse, with the relevant post-registration qualification, and have some experience of the processes and pathways associated with complex patient discharges from acute care.

You will be able to build effective relationships with both patients and all members of the MDT team and colleagues in social care, CCGs and community services. You will have the opportunity to experience many of the clinical specialties providing care at the Trust through caseload management of patients. You will be expected to work independently and creatively as part of a team and have the expertise to prioritise a busy workload.

In return we can offer you the opportunity to work as part of a dynamic specialist nursing team in a modern working environment, with peer support and access to excellent educational opportunities, from both in-house studies to degree and masters programmes. This is a challenging and rewarding post where you will be given every opportunity to develop and reach your potential as well as increasing your knowledge base associated with the many pathways of integrated discharge.

The post works full days Monday to Friday; these posts do not attract any out of hours pay enhancements.  The post is patient facing and you will be required to wear a uniform.
