Senior Discharge Co-ordinator An opportunity has arisen within the centralised discharge team based at Denmark Hill for a motivated and skilled individual with the relevant acute clinical knowledge to join the team as a Senior Discharge Coordinator and promote effective discharge planning for patients. You will be responsible for coordinating the safe and patient centered discharge of patients from the wards across Denmark Hill.  You will work with a wide range of clinical and non-clinical staff including the MDT and be vital to ensuring discharge of patients is communicated effectively to all involved in the process. You will be a registered nurse or social worker, with the relevant post-registration qualification and extensive clinical experience. You will have knowledge of the internal and external partners involved in discharging patients and be adept at communicating complex social and health information in a variety of formats. You will be able to build effective relationships with both patients and all members of the multi-disciplinary team. You will be supported by a Senior Social Worker and Discharge Coordinator Manager and the strong team of discharge coordinators and clinical administrators. There may be an opportunity to be the designated line manager for one of the Band 6 Discharge Coordinator roles in the team. You will have the opportunity to experience many of the clinical specialties providing care to patients at the Trust through caseload management of patients. You will be expected to work independently and creatively as part of a team and have the expertise to prioritise a busy workload. This post is part time,  working 7.5 hours per 3 day week Monday to Wednesday. The post does not attract any out of hours supplement and is based at Denmark Hill. The post is patient facing and you will be expected to wear a uniform.
